Adults - Diagnosis and/or medical management $399. Teens - Diagnosis and/or medical management WITH MindPrint Cognitive Assessment $549.

90-min in-person evaluation, review of required documents requested prior to evaluation (including completed questionnaires by teachers and primary care records). For ages 13 to adulthood.

Minors who have already completed full neuropsych evals first appointment only $350.

Executive Function (Life Skills) Coaching

In-depth evaluation with personalized strategies focused on improving executive functioning. MindPrint cognitive testing packages for teens improve success rates and “buy in” from teens. Currently only coaching patients aged 13 and older individually.

QbTesting $185, $150 for current patients

QbTest is an FDA-approved clinical test for ADHD that aids in diagnosis and treatment. This test can be done with a referral from your PCP, with results sent directly to them, or as an existing client of ADHD Life Medical Associates with a follow-up appointment for interpretation and treatment planning. Ages 6-60 years old.
