Comprehensive ADHD Services for Adults and Teens

Adults (19+) - $399 (includes Creyos Cognitive testing to provide objective, data-driven insight into attention, memory, and executive function—supporting a more accurate, personalized ADHD evaluation)
Teens (13+) - $549 (includes MindPrint Cognitive Assessment with data-driven insights + access to its interactive learning platform with personalized, research-based strategies)

A comprehensive 90-minute, in-person ADHD evaluation focused on clear answers and practical next steps.

To make your visit worthwhile, we require key forms and records completed before your appointment (including questionnaires and, when appropriate, teacher feedback and primary care records).

You’ll leave with:

• A clear diagnostic impression (ADHD vs. other possible explanations)

• Personalized treatment recommendations (behavioral strategies, coaching, and/or medication options when appropriate)

Minors with completed neuropsych evals (first appointment only): $350
If you can provide prior testing records, we’ll review them and focus the visit on interpretation, diagnosis clarification, and next steps.

Executive / Adult / Parent / Teen Coaching (45 minutes): $185 initial, $150 ongoing

Executive Function Based Coaching for ADHD, Organization, and Real-Life Performance. This is skills-based coaching for the day-to-day stuff ADHD makes harder: planning, follow-through, organization, time management, routines, motivation, and school/work systems.

What coaching looks like:

• An initial evaluation to identify your biggest friction points

• Practical strategies you can use immediately

• Simple accountability and systems that work with ADHD (not against it)

Teen Gold Package: $550 (includes 3 coaching sessions + MindPrint cognitive testing)

This coaching package is designed to help teens build the executive function skills they need to succeed in school and daily life—without relying on constant reminders from parents.

We incorporate MindPrint learning assessment platform, a research-based cognitive assessment that identifies how your teen learns best across areas like memory, reasoning, and processing. This insight helps increase buy-in, reduce frustration, and guide more effective, personalized strategies.

Instead of using trial-and-error approaches, coaching is tailored to your teen’s unique cognitive profile—helping them develop practical systems for organization, planning, and follow-through that actually stick. (Available in person and virtual)

Executive Performance Kickstart: Introductory Rate: $650 - limited availability (includes 3 coaching sessions + Creyos cognitive testing)

Executive Performance Coaching is a high-level, data-informed coaching experience designed to optimize how you think, plan, and perform. Grounded in executive function science, this service goes beyond traditional ADHD coaching by integrating Creyos cognitive testing platform to objectively measure cognitive skills like attention, working memory, and reasoning.

This deeper level of insight allows us to tailor strategies with precision—helping you build systems that are not only effective, but sustainable. Instead of guessing what might work, we use your cognitive profile to guide targeted interventions that translate into measurable improvements in real-world performance.

Coaching is currently offered for patients age 13+ (individual coaching). Parent coaching and adult coaching are available.

In-Person or Telehealth: $150

For established patients who are already under our care. Medication management is available for ADHD, anxiety, and depression.

Follow-ups are typically needed every 3-4 months to monitor response, adjust dosing if needed, and make sure your plan is still working for your real life.

$350 (Ages 8–21)

MindPrint is completed online at home and includes a 1-hour consultation to review results and walk through personalized recommendations.

Optional (when appropriate):
504 / IEP recommendations for school: $99

$185 ($150 for current patients) for anyone ages 6–60.

QbTest is an FDA-approved clinical test that can help support ADHD diagnosis and treatment decisions by measuring attention, impulsivity, and activity levels.

Available:

• With a referral from your PCP (results sent directly to them) or

• For existing ADHD Life patients (with a follow-up visit for interpretation and treatment planning)

GeneSight is a genetic cheek-swab test that shows how your genes may affect medication outcomes. It can help reduce trial-and-error by identifying meds that may be more likely to cause side effects or be less effective for you.
